For decades, creatine lived in a very specific corner of the wellness world. It was the thing serious athletes took. The powder in the gym bag. The supplement associated with lifting heavy, building bulk, and chasing performance gains. 

That reputation wasn’t wrong, exactly. But it was incomplete – and it kept a lot of people from paying attention to research that’s actually highly relevant to them. 

Because it turns out that some of the most compelling science on creatine isn’t about athletic performance at all. It’s about what happens to muscle, energy, and cognitive function as we age – and why women, in particular, may have more to gain from creatine than almost anyone. 

So, What Actually Is Creatine? 

Creatine is a naturally occurring compound your body makes on its own, synthesizing it primarily in the liver and kidneys from three amino acids: arginine, glycine, and methionine. From there, it’s transported through the bloodstream and stored mainly in skeletal muscle – with smaller amounts in the brain and other tissues. 

Its job is energy. Specifically, it supports the rapid regeneration of adenosine triphosphate, or ATP – the molecule your cells use as fuel for virtually everything they do. 

Here’s how that works in practice. Your cells can only hold a small amount of ATP at any given time. The moment a muscle contracts – whether you’re lifting a weight, walking up stairs, or standing up from a chair – ATP gets used up almost instantly. Your body needs to replenish it just as fast. Creatine, stored as phosphocreatine in muscle tissue, donates its phosphate group to regenerate ATP in real time. The more phosphocreatine available, the more efficiently your cells can keep pace with demand. 

That process matters not just in the gym, but in every physical activity you do throughout the day. And because the brain is one of the most energy-intensive organs in the body, it applies there too. 

Why This Goes Well Beyond the Gym 

The athletic use case for creatine is well established. But the research that’s emerged over the past decade has expanded the picture significantly. 

Lean muscle maintenance. After around age 30, the body begins gradually losing muscle mass – a process called sarcopenia. By the time women reach their 50s and 60s, this can become a meaningful factor in strength, mobility, and physical independence. A meta-analysis published in the Journal of Strength and Conditioning Research found that creatine supplementation in older adults was consistently associated with greater improvements in lean muscle mass and strength compared to exercise alone [1]. The mechanism is straightforward: more available ATP means muscles can work harder, recover faster, and maintain their mass more effectively over time. 

Energy at the cellular level. Fatigue that feels disproportionate to what you’re actually doing is a common experience after 40. Part of that comes down to declining cellular energy efficiency – cells that are slower to regenerate ATP simply have less fuel available for the work at hand. Creatine directly supports ATP regeneration, which is why its benefits aren’t limited to exercise recovery. Everyday energy – the kind that carries you through a full day without the mid-afternoon wall – runs on the same system. 

Cognitive function. This is perhaps the most surprising area of creatine research, and the one attracting the most attention from scientists right now. The brain uses ATP at a remarkable rate, and cognitive tasks – processing, recall, focus, problem-solving – are all energy-dependent. A 2021 review published in Nutrients found that creatine supplementation was associated with improvements in memory and cognitive performance, particularly in older adults and under conditions of mental fatigue or stress [2]. A separate study found that creatine improved working memory and processing speed in healthy adults [3]. 

Why Women Over 40 Should Be Paying Particular Attention 

Here’s something that rarely gets mentioned when creatine comes up: women naturally have lower creatine stores than men. 

Research suggests women’s baseline muscle creatine levels are roughly 70–80% of men’s [4]. That gap matters, because it means women are starting from a lower baseline – and have more ground to make up through supplementation. 

The menopause transition amplifies this further. Estrogen plays a role in muscle maintenance, energy metabolism, and even cognitive function. As estrogen levels decline during perimenopause and post-menopause, women often experience accelerated changes in muscle mass, energy levels, and mental sharpness – precisely the systems that creatine is most well-positioned to support. 

A 2021 review specifically examining creatine’s relevance for women noted that supplementation appeared to be particularly beneficial for muscle function and strength in post-menopausal women, and highlighted its potential role in supporting cognitive performance during the menopause transition [5]. 

The “won’t it make me bulky?” concern is worth addressing directly. Creatine doesn’t build muscle on its own – it supports your body’s ability to maintain and use the muscle you already have. Combined with regular activity (which doesn’t have to mean intense training), it helps preserve lean muscle that would otherwise gradually diminish with age. What most women notice isn’t added size. It’s feeling stronger, less fatigued, and more capable in their everyday movement. 


 


Creatine Beyond Muscle and Energy 

The studies on creatine and older adults paint a consistent picture. Beyond muscle and energy, researchers have examined its effects across a range of outcomes relevant to healthy aging. 

Bone health has emerged as a meaningful area: a 2015 review found that creatine supplementation, in combination with resistance training, was associated with greater improvements in bone mineral density in older adults compared to training alone [6]. Given that bone density becomes a priority health consideration for many women post-menopause, this is a notable finding. 

Recovery is another area where the research is clear. Creatine supports faster restoration of muscle function after physical exertion – which matters considerably as natural recovery windows lengthen with age. Less soreness, quicker return to full capacity, and more consistency in staying active over time. 

The Form Matters 

Not all creatine supplements are the same. Several forms exist on the market – creatine hydrochloride, creatine ethyl ester, buffered creatine, and others. But creatine monohydrate remains the form with by far the most research behind it. Decades of studies, across diverse populations, using creatine monohydrate as the study compound. 

When evaluating a creatine supplement, the questions worth asking are simple: Is it creatine monohydrate? Has it been third-party tested for purity? And is the dose consistent with what the research actually used – which, across most studies, falls in the range of 3-5 grams daily? 

A Supplement Whose Time Has Come 

Creatine has been one of the most studied compounds in nutritional science for over 30 years. What’s changed isn’t the science – it’s the conversation around who the science is actually relevant to. 

Women over 40 managing the compound effects of aging, hormonal change, and the daily demands of active life have as much to gain from creatine as any gym-going athlete. The research makes that case clearly. And as more women discover that the “bodybuilder supplement” was quietly doing something much more interesting all along, the demographic taking creatine is shifting to reflect exactly that.
